assistance with banking, insurance claims and Medicare

forms, living wills and medical powers of attorney.

EQUIPMENT RENTAL
Durable medical equipment such as walkers, wheelchairs,
canes, potty-chairs,

IN HOME/PERSONAL COMMUNITY CARE
Provide
personal care, food preparation and light housekeeping to
at-risk
frail, elderly to enable them to remain in their homes as for long
as possible.
Medicaid covers the fees for eligible participants.
Non-Medicaid
clients pay a fee based on a sliding
scale according to
income and availability of services
